What Is Transition Support?
Transition Support is a short-term, focused service designed to help seniors safely return to their daily routines after a hospital stay. Our Personal Support Workers (PSWs) provide practical assistance, emotional reassurance, and structured routines to help prevent complications that often lead to hospital readmissions.
Our Transition Support services may include:
Creating and supporting a recovery-focused care plan
Our Care Coordinator will work with you, your family, and your medical team to understand post-discharge instructions and build a safe, personalized plan.
Assistance with personal care and daily routines
Bathing, dressing, grooming, mobility support, and safe transfers.
Meal preparation and household help
Light housekeeping, laundry, meal preparation, grocery pick-ups, and errand support.
Medication reminders
Helping ensure medications are taken correctly and on schedule.
Transportation and accompaniment to follow-up appointments
We help ensure no appointments are missed and that important post-hospital instructions are followed.
