Accident Incident Report
This form below is to be used in the event of a workplace injury. If you are the injured worker – seek medical attention first, then let your service coordinator know you are hurt. You or your Solus service coordinator will complete the form. Your service coordinator will then make sure this form is completed and sent to the Solus office within 24 hours.
